Patient Communication Consent
Apex Heart Care of North Texas uses multiple communication methods to ensure timely and effective delivery of medical care.
Communication Methods
With patient consent, we may communicate using:
- Phone calls
- SMS / text messages
- Patient portal messages
These communications may include appointment reminders, care instructions, follow-up messages, test coordination, and administrative notices.
SMS Consent Language (Website & Forms)
When applicable, patients may be asked to provide SMS consent using language such as:
"By checking this box, I agree to receive SMS messages from Apex Heart Care of North Texas related to appointment reminders and care coordination. Message frequency may vary. Message and data rates may apply. Reply STOP to opt out. Reply HELP for assistance at 940-312-6262. See our Privacy Policy and SMS Terms & Conditions."
- SMS consent checkboxes are optional
- Checkboxes are unchecked by default
- Consent is not required to receive medical care
Patient Rights
- You may opt out of SMS communications at any time by replying STOP.
- You may update communication preferences by contacting our office.
- You may choose not to consent to SMS and still receive care.
